Blood Pressure Self-Monitoring
The Y's Blood Pressure Self-Monitoring gives participants the knowledge and mentorship to monitor their blood pressure and track changes.
How it Works
Participants will work with trained Healthy Heart Ambassadors for the duration of the four-month program. During this time, participants will be encouraged to: -Self-measure their blood pressure at least two times per month-Attend two personalized consultations per month-Attend monthly nutrition education seminars Participants will record their blood pressure readings using an easy-to-use, self-selected tracking tool. The Y will provide options for participants who need a home blood pressure monitor.
-Reduction in blood pressure-Better blood pressure management-Increased awareness of triggers that elevate blood pressure-Enhanced knowledge to develop healthier eating habits
To Qualify, Participants Must:
-Be at least 18 years old-Be diagnosed with high blood pressure-Not have experienced a recent cardiac event-Not have atrial fibrillation or other arrhythmias
Ongoing registrations and monthly nutrition seminars offered at our Nir Family YMCA.Healthy Heart Ambassadors office hours: Mondays 8am-10am, Wednesdays 12-3pm Nutrition Seminars: Sep. 17th 6:30pm-7:30pm, Sep. 24th 12:00pm-1:00pm, Oct. 22nd 12:00pm-1:00pm, Nov. 19th 12:00pm-1:00pm, Dec. 17th 12:00pm-1:00pm, Jan. 21st 12:00pm-1:00pm.
