Healthy Living With Diabetes

Get the Support and Guidance You Need

Our Healthy Living with Diabetes program is led by a trained and certified Level II Diabetes Educator and Certified Health Coach. This program is designed to provide education and support to participants who have been diagnosed with Type 1 Diabetes or Type 2 Diabetes.

Educational topics will include nutrition and cooking, physical activity, coping, stress management, and developing and maintaining healthy lifestyle changes, adapted from the AADE7 Self-Care Behaviors along with coach support and peer support through group-based classes.

Evidence shows that maintaining a healthy lifestyle while living with diabetes will help individuals reduce blood sugar and blood pressure and maintain a healthy weight. This also reduces risk factors of secondary health risks including kidney disease, retinopathy, neuropathy, and cardiovascular disease.

How It Works

This is a 3-month group-based program that includes weekly sessions that are 1.25 hours. This program will be led by Diabetes Educator and Certified Health & Lifestyle Coach, Bridget Carroll. Participants will learn how to implement healthy lifestyle habits and improve self-management of their diabetes. Participants are expected to attend all or as many sessions as possible and track their food, physical activity, and blood sugars daily and weekly, and submit weekly to Coach Bridget.

Program Goals

To educate and empower patients living with Type 1 or Type 2 Diabetes with managing their chronic condition through a healthy lifestyle and will have improved blood sugar levels, blood pressure levels, healthy weights, and gain more understanding of nutrition & cooking, and importance of physical activity.
